Why medical and strength insurance contained by malaysia doesn't cover cosmetic surgery?

Why would you expect cosmetic surgery to be covered?
Its not medically mandatory, by definition. Just cosmetic. If you want it done, save up the money. If you don't want to/can't save up the money, consequently do without.
