It's organization policy in some doctors office to bring back a copy of your insurance card whenever you visit so they can make sure the insurance hasn't changed. Just ring the office before your appointment and describe them you have lost your card and ask if they actually stipulation the card. They will tell you yes or no and if they need it, you will have need of to contact your insurance company. They may be willing to fax something to the doctors office until you draw from your new card.
Depends on their policy. But check your insurance company's network site to see if you can request a new card or print on on-line.
If the subscriber's ID number and group number have not changed, the dr's department should be able to verify your eligibility and the billing address directly with the company by phone or on-line.
~~To save you from being placed within the back of the line at the doctor's while they try and sort things out, it would be best to phone call your insurance company, tell them you lost your card and then enjoy them give you the following information:
Subscribers name, your ID number for the card, the group number, and after be sure you get the claims billing address. Also write down the 800 number to give the doctor's bureau too. Effective date for your policy.
Some insurance companies will also fax over a copy of your information directly to your doctor, so call the doc first and ask for their fax number.
This way it will solitary take a second should they need to verify anything, and you won't sit an extra hour or two while others help yourself to your place being seen!~~
If your coverage hasn't changed, you're fine, they probably won't even ask for the card, they'll just ask if coverage have changed. If they want proof, they can always call the owner and look you up using your SS#. Source(s): Health broker 19 years. www.HutchingsInsurance.com
