"What can I do around my insurance company not wanting to reimburse for some of my medical bills?

I have a $3,000 deductable. I have just this minute met that because I had surgery on my achillies tendon. Some stuff they wage for, some stuff goes towards my deductable some doesn't. How can find out if they should pay for it or not. How can I confrontation it?
