I be within a saloon twist of fate, and the insurance company wishes me to sign a w-9 form beforehand getting clearance?
the accident was near somebody in a company vehicle, and this is what they said. is this normal, or is it shady? also on the form, am i supposed to check "exempt from backup withholding" or not. thankfulness

I have been dealing near auto accidents for over 25 years as an insurance agent in Texas. I hold yet to see the W-9 form used in settlement of a claim. W-9 forms are generally usued by employers to show income....looks to me like the insurance company is trying to know how to "write this off as payroll" paid to you.
I expect that before I signed anything, I would contact your State's Department of Insurance. They probably have a toll-free number - simply call and ask for Consumer Complaints division and they should be able to assist you.

Common practice. Unless you have been told by the IRS that you are required to hold backup withholding, you are exempt.
Its just a tax statement which will show that you have income. you adjust your taxes for the loss in value for your coup¨¦. Talk to your tax advisor.
