Can you chose if you want to use your strength insurance?

I am having to come out of pocket over $400 for 1 physical therapist appointment. I surmise it will be less if I just reimburse the office upfront. Can I ask that they not go through my insurance company??

You completely can ask them to do so. Just be sure your primary Dr gets the reports from them so it is all within your medical records or ask them to provide you with a copy of the resume from each visit so you can distribute them to your Dr.
Some PT offices do give you a price break if you settle up cash and pay it 100% up front. But some of them are not relating you the full price with insurance as they are willing to adopt the amount the insurance pays and your deductible and co-pays as full payment for services rendered. Also, if you ever have to be in motion back will the price be the same again or will you enjoy satisfied your deductible for the year and it will only cost you your co-payment subsequent time? Consider this too before you make a decree.
So, check with them first to see if there is a price break for bread payment.
