Unfortunately if depends on your insurance companies policies. A worthy place to look is http://www.obesityhelp.com/. People post there insurance information (regarding weightloss) and you can look it up. However, you will still have to progress threw an extensive approval process.
I was self Pay, so I was fortunately as I didn't own to deal with approval. My insurance essentially told me NO WAY, NEVER, PEROID. I had to cash out my 401K, but I figure if I didn’t do something I wouldn’t be around for retirement anyway. However, I would pay it again in a heart overpower.
I had the band put within on May 12, 2008. It was the best thing I enjoy ever decided to do in my vivacity. I'm down 67lbs. I went from a BMI of 57 to 49 after 4 months (3 months after surgery and 1 month before). I have have to stop taking my blood pressure meds completely. I have had to curtail my CPAP from 15 to 13 then to 11. I went from taking Percocet (OxyContin and Tylenol) for my knees to taking Advil. I even walk the “ Big Dam Bridge ” this weekend, where I live (sorry I didn’t name the thing) It’s a 1/2 a mile long walking trail 600 foot high over the Murray Lock and Dam. I would never have be able to do that before. Even if I never lost another pound, I would do it adjectives over again, but I would be disappointed if I didn’t loose more weight. I have merely one regret, I’m a “I wont do this until I have all the facts” mode of guy. It took me a year to get over my own fear and to grasp the money together, before I got the belt. I regret wasting a year that I could have been losing counterweight. Source(s): http://www.obesityhelp.com/

You can go to the website Obesityhelp.com and look under the tab of finding a bariatric surgeon and look up your state and the information on doctors, insurance and reference from other patients.
I had lap group done 4-16-08, I am down 45 pounds and lots and lots of inches. I do water aerobics 3-4 times a week. I am feeling better everyday. My insulin be eliminated and back on oral medication, my blood pressure medicine has be cut in half. I enjoy orthopedic problems and they don't bother me, since losing the weight (I have two knees replacements and a metal rod and screws in my back) I lately feel better. I still have a long method to go, but this was the best for me. Good Luck
