If I pay $115 a month for health insurance and run to the doctor 3 times in one month, and a little bit over the subsequent few months, does my premium go up?

As long as you hold on to that policy, your rates will only ever go up, when they bump up the rates of EVERYONE in your class - or when you have a birthday, and are ripened enough to jump up to the subsequent class.
