Are you referring to the replacement to the E111?
If you are referring to the "E111" form, this no longer exists.
Go into a Post Office and ask for the form to apply for a EHIC (European Health Insurance Card) which is what it was replaced with - this will cover you for accident whilst in any EU country (but read the small print)
Note that it may take up to a month for the card to arrive at you once you have sent off the form.
