Free national robustness insurance: Can doctors still be within private practice, outside the organization system?

In England, Canada, etc., what happens if a doctor refuses to see elected representatives patients? Does he lose his license? Or are doctors forced to work for the national health system, as government organization? If a patient could afford to pay the fees of a private doctor, I'm wondering if within would be any in private practice and therefore whether the merciful would have that choice. I would love to hear how these matters are handle in any country with nationalized tablets, and hear from patients, whether satisfied or dissatisfied. Also I'd like to know if a lenient can still buy his own health insurance--that is, whether it's available. Or does nationalized medicine extinguish that part of the insurance industry? Thanks.

I'd like to know how these nations promise with non-citizens. We have tens of millions of non-citizen immigrant in the US. So far, no one is seriously conversation about what we will do with these ethnic group is we switch to a single payer system. Consequently, how will we deal with the millions more that will flood across the border for free healthcare, none of whom will retribution for it, US taxpayers footing the bill.
the doctor is a private person...the policy just pays for you to go to him, they don't grant the doc a salary. he charges the patient and the policy pays the patient bills.

In a hospital is different, they are given a giving a salary and such, cuz they work for the hospital, which the government owns

Doctors don't have to see NHS patients however simply 3% of GP's operate outside the NHS and most of these work for non-profit making private medical group BUPA.
