No one has answered your question but I did want share something beside you. I know of very few insurances that cover dental implants. But did you know that you do not own to get dental implants per missing tooth? See my avatar? I enjoy 8 implants on top and 6 on the bottom that holds a whole piece prosthesis to be exact attached to the dental implants with screw. I do not remove them to even clean them, nor does the dentist when I go for a cleaning.

Many employer sponsored dental plans cover implants. I had two - Aetna and Metlife. It depends if your employer includes it contained by the coverage.
Some do, most don't. They would rather cover preventive care and minor
procedures similar to cavities, crowns, bridges, etc.
As far as covering all teeth - no exception is made to any one tooth but since most employer provided insurance cap care at 1,000 - 1,500 per year (barely covers one) it would take you oodles decades to have an insurance policy cover full replacement of teeth with implant.
I had Dental Implant surgery almost six months ago and it changed my life. There are fine dentists contained by India. I have had nought problems or complications..
